A free, 12-minute assessment that shows hospitals how well they convert their safety capabilities into safer care.
HYDERABAD, INDIA. To mark World Patient Safety Day on 17 September 2026, Medblaze has launched the Patient Safety Maturity Assessment 2026, a free online assessment that helps hospital leaders answer a simple question: how safe is your hospital?

Capability is not the same as maturity
Accreditation, bed strength, a dedicated Patient Safety Officer and modern hospital information systems tell us what a hospital has. They do not, on their own, tell us how safe its patients are. The assessment keeps the two apart: it measures the hospital’s safety infrastructure and, separately, how effectively that infrastructure is used to prevent harm, learn from incidents and anticipate risk.
What matters is not only what a hospital has, but what it does with it.
What the assessment covers
After a short profile of the hospital, the assessment asks 50 core questions across seven domains:
- Leadership and governance
- Safety culture and staff engagement
- Incident and Good Catch management
- Investigation and organisational learning
- Proactive risk management
- Infection and clinical safety
- Digital safety and safety intelligence
The questions adapt to each hospital. A hospital with a dedicated Patient Safety Officer, digital incident reporting or a safety dashboard is asked how well those capabilities are actually used, while a hospital relying on manual processes is asked about the practices it runs today.
Results the moment you finish
Participants receive their results immediately, including:
- An overall maturity score and level, from Reactive to High Reliability
- A Safety Capability Index and a Safety Practice and Culture Index
- A Safety Intelligence Score, showing how well safety data turns into action
- The capability–execution gap, and a benchmark calibrated to the hospital’s size and accreditation maturity
- Three priority improvement areas, with practical next steps
Hospitals that want to go further can sign up for a demo of Medblaze Infini and receive a complimentary Patient Safety Maturity Report.
